ENH: Add notebook example of Elastix-MONAI affine bridge

Includes an example of Elastix-MONAI affine bridge that was removed from https://github.com/InsightSoftwareConsortium/itk-torch-transform-bridge/pull/6.

It will be updated/repurposed when https://github.com/Project-MONAI/MONAI/pull/5897 is merged into MONAI.
