Update Feature Request: Build out Family Plan Profile Fields of Entry

Build out the user profile fields as shown in the wireframes below. This includes, adding photos, filling in text boxes, and checking boxes for certain options.

Create expandable and collapsible sub categories for the fields. For Example "general information" can be expanded to show all of the fields within or be collapsed to hide this information.
