Add prior work from jduteaeu/brettmarquard in new dir, add section/entry

This is a replacement of the prior PR of: https://github.com/HL7/C-CDA-Examples/pull/126

It moves it out of the "guide examples" and into new folder since not currently part of the new C-CDA publication.
