Crash on login and other misc bugs

First off, I've noticed that logging in occasionally causes the server to crash, and this is what the console says when the server crashes.

Second, I've noticed that other players receive the error "Unable to retrieve ponies from database" in the character creator when they try to log in after I apply an update to my server. The only workaround to this problem I could find is to delete everybody's ponies from the database.

Third, I've noticed that the database won't save a pony if it has the same name as another pony, even if both have different designs. All the error message says is "Error while saving pony".

And last, on rare occasions, the portals don't work.

I've noticed that using patched.dlls does reduce the amount of times the server crashes when someone connects to it, but doesn't completely eliminate the problem.
