add parameter to append existing columns in VCF to output files in variant effect prediction

From discussion with @aaronkw & others. This will be an optional parameter that results in added column outputs for TSV/HDF5 labels file in variant effect prediction. Users can specify whether they want all existing columns in the input VCF to be appended to the resulting output file.
