add server to jquery csdk example

Hey! I submitted issue #18 regarding the problem loading the local image. I've added the server and updated the readme for this jquery example to help out other users. Let me know if it's useful!

I also made a plain ol' vanilla-js web example using the image editor. Happy to provide a PR for that if you are interested.
