Label annotation are missing

When ever i'm annotating the image using the objects after finishing the annotation are done when i go to check what are the labels there are few missing label i.e if i label CAT in the annotation xml i'm getting CAT__ sometimes or CA

Could you give more info on this? I have been using LabelMe for quite sometime I have never found this issue. Is it reproducible?
