Session Manager does not work nor does it appear in the toolbar

What happened?

After upgrading Classic to G4 and starting G4, Session Manager did not offer to restore the previous (Classic) session.

Session Manager appears as an extension and is enabled.

Session Manager does not appear on the toolbar.

SM is not available in the toolbar customization dialog.

Disabling SM, closing Waterfox and re-enabling SM, does not result in SM working.
