Remove TestSetup.h from unit tests.

The issue here is that we're still not fully transitioned to gtest. Need to replace TEST_CASE with TEST.

The bulk of this was fixed by dda5ee875d13bcc643cc463a8e34998729598c4c, although we still include TestSetup.h for scaffolding.
