update jquery-timepicker, add timezone option

This updates the jquery timepicker addon to 1.4.3 from 1.2. The latest version required jquery-ui-slider be loaded. I also re-generated the "smoothness" jquery-ui theme to include the "slider" styles.

Lastly, I changed the time format to include timezone.
